I am an Internist in Helsinki with a largish out patient clinic, mainly
cardiovascular and much hypertension. If I see someone I suspect to have
white coat hypertension (wch ), I measure the pressure up to 10 times and look
for a tendency of the pressure to drop. Many a time the patient gets normal
readings after some seven measurements and is likely to have wch.
ECG is a rapid and helpful tool looking for signs of LVH, but in a population
like ours a lot of people have athletes heart which looks like LVH.
The golden standard is ABPM. Also do not forget that wch is a cardiovascular
risk factor, specially for later developing hypertension. So these people
should be followed up.
